Health & Wellness Programs


The Health & Wellness Program focuses on health promotion and disease prevention education and activities for older Pennsylvanians.  The overall goal is to promote optimum health and well-being with a special focus on health risk screenings, nutrition, chronic conditions, fall risk, exercise and strengthening to prevent fall-related injury, behavioral health and medication management.

Health & Wellness programs are offered at various public locations in the community including in senior community centers, senior housing facilities, churches, physician offices, rehabilitation hospitals and more.

The mission of the Pennsylvania Department of Aging’s Health & Wellness program is to promote healthier lifestyles among older Pennsylvanians so there is measurable improvement in their quality of life and a subsequent reduction in overall healthcare costs. There are seven evidence-based Health & Wellness programs currently available in Berks County. They are Chronic Disease Self-Management, Diabetes Self-Management, Chronic Pain Self-Management, Healthy Steps for Older Adults, Healthy Steps in Motion, A Matter of Balance and the Arthritis Foundation’s Walk with Ease program
