Berks Heim delivers quality care and a comfortable, secure environment for our residents. We offer long term skilled nursing care, memory care and short-term rehabilitation. Our goal is to work alongside each resident to help them achieve their therapy and nursing goals.
Services
Nursing Care
Berks Heim has a team of Registered Nurses (RN), Licensed Practical Nurses (LPN) and Certified Nursing Assistants (CNA) who provide 24-hour nursing care. The nursing team works with each resident to develop an individualized care plan and works alongside others at Berks Heim to work on meeting those goals.

- Restorative Nursing Care: Nurses and Certified Nursing Assistants with specialized training in rehabilitation techniques work with our residents to promote independence. This team utilizes their expertise to implement elements of the individualized care plan that help our residents achieve their care plan goals.
- Registered Nurse Assessment Coordinators: Berks Heim has a team of Registered Nurses who organize assessments to promote the effectiveness of residents' individualized care plans. They also work closely with insurance companies to ensure insurance coverage for the services provided by our professionals. They are a vital member of the nursing team and advocate on behalf of our residents.
Physician Services
Robert Brackbill, MD serves as the Berks Heim Medical Director. We have attending physicians who manage the medical needs of our residents.
Rehabilitation Services
Berks Heim has a designated Rehabilitation Unit designed for those who require therapy following a surgery, illness, or injury. The goal of our short-term rehab program is to assist individuals to achieve their personal goals based on their individual care plan.

Clinic Services
Clinic Services coordinates the medical care aspects of our residents’ care. They schedule and arrange transportation for our residents to be treated by medical specialists in the community. They also coordinate in-house clinics, where our residents can receive care by medical specialists in our facility. 360 Care provides the following care at the Berks Heim Clinic.

Social Work Services
Every resident at Berks Heim is assigned a social worker. This dedicated team of professionals serves as a liaison between families and the facility, and advocates on behalf of our residents and their families. The social worker will assess resident needs upon admission and throughout the resident’s stay at Berks Heim. Social workers also coordinate the Resident Care Conferences, help residents to adjust to living at Berks Heim, facilitate the discharge planning process and connect residents to needed community resources.
Registered Dieticians
Berks Hiem’s Dietary team is led by a dietician registered through the American Diabetic Association. Our dieticians work closely with residents, their families, and the multidisciplinary care team to address any dietary needs or concerns.
Activities Department
Our Therapeutic Recreation and Activity Department provides a broad range of individualized and group activities along with periodic special events for the residents' enjoyment. Berks Heim residents are invited to share any specific requests with their activity aide. Our goal is to provide individualized, life-enriching activities.
Chaplain Services
Berks Heim Chaplains are extremely blessed to have the honor and privilege to attend to resident’s individual spiritual needs. Our chaplains are licensed and ordained in their faith orientation and Clinical Pastoral Education (CPE) certified. Communion is available upon request. Eucharistic Ministers offer Holy Communion for Catholic residents. Berks Heim Chaplains offer one-on-one visits, individualized care and spiritual encouragement. Every resident is treated with love, respect, and dignity in their home away from home.
